UNIVERSITY UNITARIAN CHURCH
YOUNG CHILDREN
(6 mos. - Preschool)
We ask families to fill out this registration form each year. This ensures we have up to date emergency and contact information, as well as important information about allergies and food restrictions. It also helps us plan appropriately sized programs and events. In this form, you can indicate which groups (Roots, World Religions, etc) you anticipate your child will participate in on Sundays. Please contact Director of Family Ministry, Aria Curtis for any questions about this form or about Family Ministry at UUC.
